Carer’s Checklist: Setting Up the Right Support
1. Social Worker or Adviser

Each borough should have a social worker or adviser assigned.

If none is assigned, contact the GP/Doctor to request help in setting up a care plan.
2. Hospital Discharge & Care Plan

If the person is admitted to hospital, they should not be discharged without a proper care plan in place.

Speak with the hospital team to make sure the care plan is secure and appropriate before discharge.
3. While in Hospital

Call the hospital daily to check on the patient’s condition and progress.

Ask to be updated about the most suitable care plan being arranged.

Remember: hospital doctors can change frequently based on shifts— keep up with regular communication to avoid gaps.
4. Appointments & Records

Keep a diary of all hospital and GP appointments.

Add reminders to your mobile phone or a visual calendar at home.

Always keep the patient aware of upcoming appointments to support mobility planning and reduce stress.
